Fit is the usual cause on a band this size. Wear it snug (not tight) a finger's width above the wrist bone, and tighten it a notch for workouts, because a loose band lets the optical sensor lose skin contact when you move. Clean the sensor and your skin of lotion and sweat. For ECG or EDA readings you must also touch the metal side edges (the button/frame) as instructed. It is a band, not a medical device — seek care for real symptoms.
